10 Hours Per Week, Hourly Rate, sign On Bonus, Mileage Reimbursement

This is a part time Nurse Practitioner/Physician Assistant role where you’ll provide direct care to geriatric patients with an average age of 85. Typically, you’ll see 4-6 patients a day for 45mins-1hr per visit. You will be conducting follow-up visits derived from hospital appointments, checking their medications, and ensuring the patient is adhering to their treatment plan. Additionally, you will review medical records, may conduct bandage changes and/or address wound care,

The point of your visit is to provide treatment for whatever the patient needs and fill in the gaps until the member can see their PCP.
You will have a panel of patients within a particular geography which will enable you to build relationships. Your patient panel is based on proximity to your home and would be comprised of members who are primarily Medicare Advantage patients

Although you are providing 1:1 care to patients, you do have an interdisciplinary team to collaborate with that consists of Physicians, Nurses, Social Workers, Pharmacists, Dietitians, Behavioral Health, Administrative Support and other professionals.
